Vaccine Protocol Update

The following vaccine protocol templates have been updated and posted on Vaccine Protocols and COVID-19 Vaccine Providers:

Moderna COVID-19 Vaccine, Bivalent (Original and Omicron BA.4/BA.5) for Persons Age 6 Months and Older
Moderna COVID-19 Vaccine, Bivalent (Original and Omicron BA.4/BA.5) for Persons Age 6 Months through 5 Years Booster Dose Only
Pfizer-BioNTech COVID-19 Vaccine, Bivalent (Original and Omicron BA.4/BA.5) for 6 Months through 4 Years (Maroon Cap)
Pfizer-BioNTech COVID-19 Vaccine, Bivalent (Original and Omicron BA.4/BA.5) for 5 through 11 Years (Orange Cap)
Pfizer-BioNTech COVID-19 Vaccine, Bivalent (Original and Omicron BA.4/BA.5) for Age 12 Years and Older (Gray Cap)
