The following vaccine protocol template is now available on the Vaccine Protocols webpage and at COVID-19 Vaccine Information for Health Professionals.

Please note there were also some minor edits made to the COVID-19 Pfizer-BioNTech BNT162b2 vaccine protocol for people 12 years and older to add the purple cap specification.
