January 31, 2021 Renewal Announcement
The renewal notice packet for practitioners whose current license will be expiring on 01/31/2021 are scheduled to be mailed out on 11/30/2020.
If you changed your address since your last renewal, and you did not notify MDH, you may not get your renewal packet. If you do not receive your renewal notice packet by December 12, 2020, please send an email to Health.SLPA@state.mn.us with the subject “Request for renewal notice.”

You can update or cancel your subscription at any time by editing your personal profile. You will need your email address and your password (if you have selected one).
