New Death Record Amendment Form
OVR updated the "death record amendment application" that has been in use since 2005. The old form had a couple of titles; you are probably most familiar with it as Application to Amend a Death Record through a Registrar. Another title was Application to Amend a Death Record through the Informant.
The new Death Record Amendment form is accessible, fillable, comes with instructions, and carries the current MDH logo. Find the Death Record Amendment Application and Instructions on County Vital Records Office Forms.
To get a Word version of the form to "customize", email the OVR Help Desk at health.vitalrecords@state.mn.us; put "Death Record Amendment Application ONLY" in the subject line.
