Updated MFAR Registration Form

The Office of Vital Records recently updated the Minnesota Fathers' Adoption Registry (MFAR) registration form.

MFAR protects the rights of putative fathers and adoptive families. A putative father is a man of any age who:

  • Thinks he is the father of a child
  • Is not married to the child’s mother
  • Has not established paternity for the child through a court or a voluntary acknowledgement form
  • Wants to be notified if the child is the subject of an adoption in Minnesota

MFAR registration is voluntary.


What did we update?

  • First, middle and last name field descriptions include the section name, for example: Child’s first name, Putative father’s last name, Middle name of child’s mother
  • Putative father’s statement (easier to read)
  • Fax number 651-201-5740
  • Forms accepted by fax or USPS only (not by email)
  • Date in lower right corner = 6/2020


The instructions and registration form print as one document. Please hand out the instructions and updated MFAR registration form to putative fathers.
