Effective immediately, please submit all patient forms to the Sage Programs via fax to 1-877-495-7545. We utilize a secure system called Esker Fax. Faxing patient forms will enable Sage to continue tracking patients in need of follow-up services and facilitate payment of claims during the Governor’s order for telework. Patient forms and claims sent via the mail will not be processed on a regular basis due to the constraints of teleworking. The change to faxing is permanent. The Sage Programs will process claims every two weeks. Paper remittances are no longer available due to teleworking. Providers should refer to the 835 electronic remittance advice (ERA) file for claim information. We highly encourage our providers to change to electronic claims as soon as possible.  

  • What does this mean for providers? If you do not have a clearinghouse, you can enroll with Utah Health Information Network (UHIN). For information about enrolling with UHIN, please contact
  • If you submit your claims via electronic 837 files, the Sage Programs will continue to send an 835 ERA file.
    • If you do not currently receive an 835 ERA file from the Sage Programs, please have your clearinghouse enroll and set that up by contacting UHIN Customer Service by phone 877-693-3071 or email  Please have them reference:

Please work with your automated payment systems to assure that patients are not billed for services due to delays caused by transitions in this unprecedented circumstance. These bills would cause unnecessary anxiety for patients during an already stressful time.
