Nominations Open for Health Care Homes Innovation Awards!
Nominations are open for the third annual Health Care Homes Innovation Awards!
If your care team (or one you know of) has achieved outstanding results on the health care home journey, tell us your story by submitting a nomination form and claim your place in the sun.

New E-Learning: Risk Stratification
HCH is excited to introduce two new e-learning courses on Risk Stratification. Learning about your Population - Introduction to Risk Stratification describes risk stratification and its link to Health Care Homes, explains why it is important, and provides examples of processes and tools. Risk Stratifying Your Population - Strategies and Examples explores models of risk stratification, highlights work being done by Health Care Homes to assess and manage patient populations and looks at the link between risk stratification and care coordination. Free and CEU eligible!
