Mother's Worksheet Available in Additional Languages

The Worksheet and instructions for creating your child's birth record is now available in Hmong, Karen, Somali and Spanish. Mothers have been using the English language version of the worksheet since late 2017.

The translated worksheets are on the Birth and Fetal Death Registration Information for Hospital Staff Forms & Reference Information and MR&C Information webpages. (I wanted to attach all four worksheets to this message but ran into technical limitations.)

Please give one of these worksheets to a mother whose first language was Hmong, Karen, Somali or Spanish. Mothers of newborns complete the worksheet to assist in completing the birth record for their child.

The translated worksheets and instructions carry the current Minnesota Department of Health logo and are accessible, but not fillable. To help birth registrars with data entry, the translated worksheets mirror the English language version of the worksheet released in November 2017.
