The annual Health Care Homes Learning Days conference brings together more than 300 health care providers, public health professionals and community partners to exchange information, advance health transformation, and build relationships. Save the date!

Learning Days Call for Proposals:   Submit by November 9, 2018

We learn best from each other!  Lead a breakout session and share your lessons learned to help others transform their practice.  Proposals requested for beginning through advanced learners on the following topics:  whole person care, team based care, community partnerships, quality improvement, health equity and value-based care.

Minnesota ranks high for quality of care and health status, but has stark disparities within the overall population.  By analyzing patient data, clinics can reveal disparities and health equity issues. West Side Community Health Services will share how they use REL data to identify and address gaps through quality improvement initiatives.
