The Infectious Diseases Society of America (IDSA) and the Society for Healthcare Epidemiology of America (SHEA) have developed new guidelines for Clostridium difficile, including new advancements in prevention, diagnostic methods and treatments for children and adults. The guidelines now include:
- Preventive strategies for epidemiology and antibiotic stewardship
- Specimen collection and laboratory testing using molecular tests vs. immunoassay
- New recommendations for antibiotics include vancomycin and fidaxomicin as the first-line treatment
- Fecal microbiota transplantation (FMT) as a recommendation for patients who have experienced multiple recurrences of C. difficile infection
- Recommendations on epidemiologic surveillance, diagnosis, and treatment of C. difficile in children

IDSA has issued new guidelines that promote the use of fecal transplants to treat Clostridium difficile. C. diff infections are associated with antibiotic use that can destroy the natural defenses of a healthy gut microbiome. In the new guidelines, the ISDA notes a success rate nearing 90 percent of transplants, although long-term data on the effectiveness of the treatment is not yet available. Fecal transplants are intended to take the microbiome of healthy donors and allow them to flourish in the recipients.

The U.S. Food and Drug Administration (FDA), along with the Centers
for Disease Control and Prevention (CDC), representatives from the American
Society for Microbiology (ASM), and other endoscope culturing experts,
developed voluntary standardized protocols for duodenoscope
surveillance sampling and culturing.
The protocols outline steps hospitals and health care
facilities can take to reduce the risk of infection and increase the safety of
these medical devices. These steps are in addition to meticulously following
manufacturer reprocessing instructions.
Find out more on this topic during a webinar
with FDA, CDC, ASM and other endoscope culturing experts on March 22, 2018, 12:00-1:30 p.m. central time.
