  • April 3 - Hear a panel on social determinants of health, attend breakout sessions, visit exhibits, and congratulate the first winners of the Health Care Homes Innovation Awards!  Close out the day with colleagues and friends at a reception hosted by the HCH nurse planners.
  • April 4 - Attend an in depth workshop to exchange information and learn take home skills.  Workshops include a special overview of how Health Care Homes and Integrated Health Partnerships are evolving, and a care coordination workshop focused on pediatric populations.

Introducing: Check Up From the Neck Up

Health Care Homes is excited to introduce "Check Up from the Neck Up", a new e-learning course for clinicians and care team members focusing on best practices and tools for diagnosing cognitive impairment in older adults. Created in partnership with Act on Alzheimer’s, this free course includes video demonstrations, webinars, and practice exercises.  Register on the new Health Care Homes Learning Management System (LMS).
