Nominations are open for the first Health Care Homes Innovation Awards! If your care team (or one you know of) has achieved outstanding results on the health care home journey, tell us your story by submitting a nomination form and claim your place in the sun.
Winners will be honored at an awards ceremony during the closing session of Learning Days on April 3, 2018, and awards will be reported to your organization’s leaders and local media. Share your success, inspire others, claim the recognition you deserve, and prove to others that Health Care Homes is health care that works!
