Minnesota law requires schools to complete a report summarizing the immunization status of all students K-12 enrolled in your school – the Annual Immunization Status Report (AISR) online reporting tool is used to complete this report. It is the responsibility of the school administrator (or designee) to complete the AISR by Dec. 1 each year.

Tips to complete the AISR:

  • Refer to the User Guide: Annual Immunization Status Report (AISR) Online Reporting Tool and the School Immunization Reporting Webinars for help completing the report.
  • All returning users will need to create a new password when you log in for the first time this year. If you receive an error message, please follow the instructions on Page 3 of the user guide.

Please make sure the school nurse for your institution receives this message.
