Consolidated State Performance Report (CSPR1) Survey

  

Each year, the federal government requires districts and states to report certain information about education programs. Much of this information is reported through the annual Consolidated State Performance Report (CSPR). The majority of the information reported through the CSPR comes from data that schools and districts report through the state’s data systems but there is some information for which the only source is a district survey. Each year MDE must collect survey responses from districts on a number of issues to meet the federal requirements of the CSPR.

The following is a link to this year’s CSPR I Zoomerang survey: http://www.zoomerang.com/Survey/WEB22D6NH5GWKX. This survey is extremely important to the completion of the CSPR. Failure to report information to the CSPR in a timely and accurate manner can have serious consequences for the State’s federal funding. It is therefore critical that each district thoughtfully complete this survey in a timely manner. For this first survey, districts must complete their responses by October 19, 2011. Once you begin the survey, you have to answer all of the questions to exit. Therefore, it is best to print the survey off, retrieve the necessary data to answer all the questions and complete the survey in one session. A printable copy is attached for your convenience.

 The CSPR is divided into two parts, CSPR I and CSRP II. MDE is only collecting data for CSPR I at this time but will soon be requesting your response to a second survey for CSPR II.
