Introducing CES Chats

Meet with the CES Planners and chat about Coordinated Entry

The Hennepin CoC Coordinated Entry Planners are hoping to "visit" with each agency that has a program(s) receiving referrals through the Hennepin CoC Coordinated Entry System (CES).  The meeting will be informal with a primary goal of checking in to see how the CES is working for you and to share data from our new provider level dashboard!  We hope to make these an annual occurrence and look forward to connecting!

Assessor Renewal training is coming October 1st

All CES Assessors will soon be receiving an email from Convene with a link to complete their annual renewal training. Please prioritize this training, as starting 10/1, you will notice some changes to the CES assessment in HMIS. The training will incorporate new changes to the assessment, as well as best practices and refreshers.

Assessors will have until the end of the calendar year to finish the renewal training. However, for two new features (Documentation Status & Client Choice series) – assessors will have 1 month (11/1) to complete those modules. 

Client Choice series update to the assessment is going live 10/1/21 

Since January, Hennepin County has been in partnership with C4 Innovations in an effort to develop a more equitable coordinated entry process that centers client choice. C4 Innovations went through an extensive process of conducting listening sessions with members of Street Voices of Change and guests at People Serving People. After synthesizing their feedback, C4 convened a CES Stakeholder Planning Workgroup that was inclusive of partners with lived expertise and frontline staff in order to review and revise the client choice series portion of the assessment process. The group met on a biweekly basis from late May through early August and provided very thoughtful feedback and recommendations in order to develop a revised series of questions, list of recommendations about the assessment and housing process, and a script that Assessors can use to ensure the intake process is trauma-informed and responsive to clients’ individual needs.

Assessors have been invited to a briefing with C4 on 9/29 to introduce the updated client choice series questions along with the new Assessor script.

Housing Providers - where to find vital documents in HMIS

As CES assessors are trained on how to assist households with obtaining vital documents (ID, Social Security Card/Proof of application, Birth Certificates) Housing Providers will find those documents in HMIS. This will speed up the the intake process to housing programs.

To find the vital documents in HMIS, open the household's assessment. Then go to the Client Profile tab (as pictured below).

Scroll down to bottom of page until you get to the File Attachments box. There you will find the vital documents.

Housing Providers - Update the CES team if you have a new point of contact

A reminder to Housing Providers that are working with CES. It is vital that the CES Team knows who the primary point of contact is for all projects. If you have change in staffing, please notify the CES Team (CES.Hennepin@Hennepin.us).
