Work Comp Campus, vocational rehabilitation and R-forms
The Department of Labor and Industry (DLI) has temporarily shut down email notices about missing or late R-forms. We understand some notices were sent in error when the requested form had already been filed with the department. Other requests were sent accurately; however, Work Comp Campus functionality has not been available. We apologize for the inconvenience.
Until Campus functionality is restored
Until Campus functionality is restored, vocational rehabilitation providers should continue to perform consultations and develop and circulate R-forms and required reports to the parties as required by the rehabilitation rules.
Filing when Campus functionality is restored
When DLI notifies rehabilitation providers that Campus functionality has been restored, required rehabilitation reports and progress records (including R-forms) must now be filed in Campus within 15 business days of the notice.
Penalties when filing in Campus is unavailable
DLI will not issue penalties against a rehabilitation provider under Minnesota Rules 5220.0130, 5220.0410, 5220.0510 or 5220.0450 for failure to file required rehabilitation reports and progress records (including R-forms) when system functionality is unavailable. DLI may assess penalties if the required rehabilitation reports or progress records (including R-forms) are not filed within 15 business days of notice that system functionality has been restored.
Nor will DLI issue penalties against a rehabilitation provider under Minnesota Rules 5220.0130, 5220.0410, 5220.0510 or 5220.0450 for previously filed rehabilitation reports and progress records identified on the list the provider has sent to DLI.
