The COVID-19 Provider Alert Survey seeks to identify challenges faced by providers that deliver the following services during the COVID-19 pandemic:
- Intensive residential treatment services (IRTS)
- Substance use disorder residential treatment
- Personal care assistance (PCA)
- Adult foster care/supported living services.
This survey helps us to better understand critical COVID-19-related pressures on providers, such as:
- The number of confirmed or suspected COVID-19 infections
- The need for PPE and monitoring equipment
- Staffing and service capacity.
Submitting information through this survey is voluntary. Decision-makers will use the information submitted about the types and scope of COVID-19 impacts among providers in order to improve systems and to inform emergency response coordination among state and local agencies.
