Grant oportunities

Check out these DHS opportunities for funding action plans:

  • Live Well at Home Grants – the FY2021 Live Well at Home grant program RFP was released on February 18 and will close on April 17, 2020. The purpose of these grants is to develop and/or provide services for older Minnesotans to live in the community. Visit the Live Well at Home grants website for more information about the grant and application instructions.
  • Disability Services Innovation Grants – the FY2021 Innovation grant program RFP is expected to be released in spring 2020. The purpose of these grants is to promote innovative ideas and to improve outcomes for people with disabilities. Visit the Innovations grants website for more information about the grant and visit the site frequently for updates on the release of the RFP.
