UPDATE FOR ERROR IN SUBJECT LINE

PURPOSE: To alert HCBS service providers and nursing facility staff to the June 8 registration deadline for the June 16, 2016, long-term services and supports quality conference.

Register before June 8 for the 2016 LTSS quality conference

If you would like to attend the 2016 long-term services and supports quality conference, "Quality Improvement Across the Continuum," make sure to register before June 8, 2016.

Please register and pay for the conference on the registration site. You can sign up to attend as an individual or as a group.

 You must register before June 8, 2016. There is no same day registration.


Conference basics

The registration fee includes:

  • Admission to all conference sessions and materials
  • Morning and afternoon breaks, breakfast and luncheon provided during conference hours
  • Networking opportunities and more!

Topics

More than 30 service providers from across Minnesota will share first-hand experiences and lessons learned from implementing quality improvement projects. Participants will leave with strategies and practical ideas to apply to their own organizations.

Select project topics include:

  • Quality Improvement 101 and data development strategies
  • Adapting an evidence-based diabetes prevention program to better support Somali elders
  • Innovative partnerships with local colleges and businesses to improve peoples’ quality of life and other communication efforts
  • Integrating mental health and home care services to provide more holistic services for people with mental illness
  • Improving infection prevention and control and other wellness programs.
