The Aging and Adult Services,
and Disability Services divisions including the Continuing Care Performance
Measurement section seek proposals from qualified responders to conduct a
multi-year project to assess, monitor and evaluate home and community-based
service (HCBS) access.
The project must:
- Develop benchmark measures for HCBS access, including but not limited to critical access
- Monitor and evaluate HCBS access
- Inform aspects of the state's "Access Monitoring Review Plan," per federal rules and state reporting requirements.
