Grant proposals sought to assess, monitor and evaluate HCBS access

The Aging and Adult Services, and Disability Services divisions including the Continuing Care Performance Measurement section seek proposals from qualified responders to conduct a multi-year project to assess, monitor and evaluate home and community-based service (HCBS) access.

The project must:

  1. Develop benchmark measures for HCBS access, including but not limited to critical access
  2. Monitor and evaluate HCBS access
  3. Inform aspects of the state's "Access Monitoring Review Plan," per federal rules and state reporting requirements.

Proposals must be time-stamped by the DHS email system by 4 p.m. Central Time, Tuesday, June 21, 2016. DHS will not accept late proposals.
