Hello VRS Community Partners and Stakeholders.
Vocational Rehabilitation Services (VRS) has developed a new accessible and fillable Monthly Progress Report – Job Placement and Follow-Up Services form.
We request you begin using this form immediately for all individuals being served under a Performance Based Agreement (PBA). Placement professionals are expected to complete and submit the form by the 10th of each month following the month of service, beginning with the Signed Placement Plan meeting and continuing through Placement Closure.
