Youth in Transition Roles
Youth with disabilities don’t navigate the journey to adulthood alone—and they shouldn't have to. From family members and guardians to school staff, service providers, and vocational rehab professionals, many people play a key role in helping youth plan for their future.
The Youth in Transition Roles guide breaks down who’s who on a youth’s transition team—what they do, why it matters, and how they can help. You’ll also find helpful tools and resources that support transition planning, like My Vault, the Pre-ETS Inventory, and more. Check it out to discover how we all play a part in supporting youth on their paths forward.
