Connect to your COVID-19 vaccine

Minnesota has a new online form that will help all Minnesotans know when, where, and how to get their COVID-19 vaccination. As vaccine supply increases and doses become more widely available, opportunities to get vaccinated will expand.

Fill out the form to stay updated on your eligibility for the COVID-19 vaccine and connect to vaccine opportunities in your area when they become available.

A Tip For Blind, DeafBlind, and Visually Impaired Users on Completing the Captcha Field

In the process of completing the online form you will encounter a “CAPTCHA” verification screen that must be completed before starting the form. This is a form that requires the user to enter the numbers and letters displayed on the screen or played through an audio file. In this CAPTCHA configuration, when you play the audio file you will hear the NATO phonetic word for the letter, rather than the letter itself. For instance, you will hear “Alpha” instead of “A,” “Bravo” instead of “B” etc. Just enter the letter itself, and not the word. (The letter you enter will correspond to the first letter of each word spoken.) Also be aware that the audio file will begin speaking immediately, so, if you are using a computer, you may wish to hit the control key to stop your screen reader from speaking over the audio.

Unfortunately, DeafBlind consumers may need assistance in completing this CAPTCHA verification. If a better solution for DeafBlind consumers is implemented, we will send out that information.

  • Signing up for the Vaccine Connector is easy, safe and secure.
  • After completing the form, you will receive confirmation via email that your Minnesota COVID-19 Vaccine Connector sign-up is complete.
  • When the information you provided through the form suggests you might be eligible to receive the COVID-19 vaccine, you will receive information on vaccine opportunities in your area and how to schedule your appointment. If you don’t have an email address, you will be notified via text message or phone call.
  • All Minnesotans 18 years of age and older who have NOT previously submitted information should sign up, no matter their current vaccine eligibility status. (If you have already received your vaccination, there is no need to sign up.)
  • There will be no cost and no restricted time period for signing up.
  • Health insurance and personal identification forms are not required for signing up.
  • Go to mn.gov/vaccineconnector to sign up.
  • Minnesotans unable to sign up online can have family or friends help them sign up online, or call 833-431-2053 for assistance signing up over the phone.
  • Translation is available by phone in all languages. Minnesotans can call the translation hotlines at 833-431-2053 and 651-318-0989 for assistance signing up over the phone.
  • All Minnesotans have to do to sign up is provide basic information such as contact information, demographic data, medical history, and employment to determine their eligibility based on state guidelines.
  • The State of Minnesota is committed to the equitable distribution of safe and effective vaccines for protection against COVID-19 and the Vaccine Connector form has included questions about gender identity, sexual orientation, race, cultural identification, and disabilities to help track and measure equity and fairness throughout the vaccine distribution process. These questions have the option to select “prefer not to answer” or “none of the above,” and selecting these answers will not prevent you from signing up for the Vaccine Connector.

Congratulations to Nawl Mohamud

Nawl Mohamud, a 7th-grader in the Saint Paul School District is the winner of the District and Regional levels of the Lions International Peace Essay Contest for visually impaired students. The theme for this year’s contest was “Peace through Service,” and the contest guidelines also encourage applicants to reflect on the current moment. They write:

“While overcoming new challenges brought on by an unprecedented global pandemic, we’re celebrating the things that keep us connected—to each other, to our communities, all together around the world. This year, we invite young people with visual impairments to explore and express these connections.”

Here is the closing paragraph from Nawl’s winning essay:

 “As I continue to experience the Coronavirus, I know I can still feel peaceful during this pandemic and I want to share this peace with everyone I see. It starts with small changes that I can create in my community like smiling, saying positive things, and sharing iPad pictures with others. These gestures are my way of showing peace through service. I want people to know that things are hard right now, but if a 7th grader like me can spread peace in small ways, so can my community. We will get through this together.”
