We are excited to share an update on the redesign of the Follow Along Program, made possible through funding from the Healthy Beginnings, Healthy Families Act. As part of this redesign, we convened two groups – the community engagement team and community connectors – to gather valuable insights and identify opportunities for program improvement. In collaboration with the Management, Analysis, and Development Division team within Minnesota Management and Budget, both groups recently completed summary reports of their work over the past year. 

Community engagement team  This group of about 30 members brings diverse languages, cultures, and perspectives to monthly meetings to inform Follow Along Program updates. A survey was completed with community engagement team members and results highlight key priorities, including: 

  • Emphasizing flexibility and individualization during program implementation.
  • Reducing the administrative burden for families of young children through a mobile app.
  • Developing strategies to connect families with resources more effectively. 

Community connectors  MDH has grants with eight organizations that represent and serve cultural groups across the state. These grantees provide culturally and linguistically inclusive support for families enrolling and participating in the Follow Along Program. Community connectors have identified several challenges and priorities for families accessing the Follow Along Program, such as: 

  • Ensuring timely communication.
  • Using plain, family-friendly language.
  • Strengthening partnerships and increasing collaboration with community organizations.
  • Better utilizing existing resources to meet families’ needs.

 MDH is working closely with local public health agencies to utilize this information as we improve program access and effectiveness.
