Apply for Transitions in Care for Justice-Involved Populations (QIF-TJI)
Who can apply
You must be a Health Center Program award recipient with an active H80 grant award to apply. This means you receive funding under Section 330(e), (g), (h), and/or (i) of the Public Health Service Act, as amended. Review the Eligibility Information in the NOFO for more details.
Get key funding details
We expect to award $51 million in total funding to approximately 51 health centers in FY 2025. Health centers can apply for up to $1 million. This is a one-time competitive funding opportunity.
Health centers will use QIF-TJI funding to strengthen transitions in care for people who will soon be released from jail or prison and return to live in the community.
This funding will increase access to community-based, high quality primary care services for justice-involved individuals reentering the community (JI-R).
Community Resource Center and Kinship Navigator Program Request for Proposal Available
Ensuring families have what they need is one of the primary goals of the new DHS request for proposals (RFP) to create a Network of Community Resource Centers and Kinship Navigator Programs. Supporting places in community where families feel safe and supported to access programs and services is a key strategy in promoting family and community well-being. This RFP is now open for application.
Potential responders can learn more about the RFP at one of two responders conferences:
April 30, 2024 at 12:00 p.m. or May 1, 2024 at 4:00 p.m.
We encourage all eligible applicants to apply by May 31, 2024.
