 Prep and Post your 2015 OSHA 300A!
It is time, once again, to:
-
prep your OSHA 300A, the annual Summary of Work-Related Injuries and Illnesses, and
-
post it in the workplace February 1st through April 30th.

Prepping your OSHA 300A

Review your 2015 OSHA log to make sure each injury/illness is correctly documented and the number of days away, restricted, or transfer are correct.

Retrieve your hours worked and average number of employees from SEMA4. MMB has run a job in SEMA4 (HP3603) which produces a blank OSHA300A for each Establishment (OSHA Location) in SEMA4. Please note: The SEMA4 300As will not have any injury data but will have each Establishments total hours worked and average number of employees. Use the following SEMA4 path to retrieve the report. On-Demand Reports>Standard Report Viewer>DocumentDirect>HP3603. Be sure to use the January 19, 2016, report. If you don't have access to the report in SEMA4, please check with others within your Human Resources office. - Retrieve a blank copy of the OSHA300A (second document of three)
- Fill in all the fields on the 300A
- Have the completed 300A signed by your agency head.
Posting your OSHA 300A
- You must post a copy of your completed OSHA 300A in each establishment in a conspicuous place or places where notices to employees are customarily posted. You must ensure that the posted annual summary is not altered, defaced or covered by other material.
- You must post your completed OSHA 300A no later than February 1 and keep the posting in place until April 30.
