Professional Services Leave certification

ORS is conducting an annual review of members who are on professional services leave or professional services released time.

Please review Section 3.02.05: Professional Services Leave and Professional Services Released Time in the Reporting Instruction Manual and complete the attached certification form accordingly. Fax the completed form by January 10, 2019 to our office using our new fax number: 517‑284‑4416.
