Topic: AFC/HFA COVID-19 Reporting Updates
Pursuant to joint Letter issued by MDHHS and LARA dated October 21, 2020 (attached), all Michigan Adult Foster Care (AFC) and Homes for the Aged (HFA) facilities with bed capacity of 13 or greater will be required to transition to a new platform for COVID-19 reporting. At this time, AFCs and HFAs with bed capacity less than 13 are exempt from MDHHS weekly COVID-19 data reporting requirements.
The Survey Gizmo platform in which all AFC and HFA facilities have been reporting was decommissioned effective October 21, 2020 and is now closed to reporting.
Transition to the new Qualtrics reporting platform will occur in two stages:
- On October 28, 2020, all licensed and operating AFC and HFA facilities with bed capacity greater than 13 must register in the new reporting tool: "Michigan AFC/HFA COVID-19 Reporting"
This initial registration survey will be active only on the required reporting date, October 28, from 7 AM to 12 PM (noon).
All registrations must be complete not later than noon on that date. Please see attached PDF for advance review of survey.
- Upon completion of the initial registration survey, each registered location will receive an individualized link to access a future weekly reporting survey within the Qualtrics reporting tool. The first weekly reporting survey must be completed on Wednesday, November 4, 2020 by 12:00 PM (noon), and on Wednesdays each week thereafter.
In an effort to support facilities in making this transition, the Michigan Department of Health and Human Resources is hosting two AFC/HFA COVID-19 Reporting technical assistance sessions. Each session will cover identical material and provide time for open Q&A on reporting in the new Qualtrics platform. We encourage staff responsible for COVID-19 reporting from each AFC/HFA facility to attend one or both sessions (attendance capacity may be limited).
We anticipate a large audience in attendance, please plan to join the zoom call 10-15 minutes before scheduled start.
