The current LHD contact information form has a flag at the top of column F that consists of two checkboxes: one to indicate the form has new information, and one to indicate there were no changes from the previous quarter. It is very important that EPCs mark that flag correctly when submitting contact information updates. Forms that have the box checked indicating there is new information are reviewed in more detail to make sure our contacts data base is up to date. Forms that have the no changes box checked are filed for compliance and audit purposes. DEPR has had several instances during this budget period where the “no changes” box was checked upon submission, and later it was discovered that new information was contained in the form.
