2021 Annual COVID-19 Surveillance Data Audit
This communication is intended for Skilled Nursing Facilities (SNFs)
Important reminders:
1. Phase I, NHSN Data Review and Correction must be complete by 5PM EST July 12, 2021.
2. Phase II, EMResource Historical Data Review and Correction must be completed by 5PM EST July 23, 2021. Please refer to the SNF EMResource Corrections Flow Chart for quick reference and note that the attestation survey and corrections worksheet for this phase of the COVID-19 Surveillance Data Audit need only be completed if corrections are identified.
