In this update:



BP3 Work Plan for LHDs & Tribes

Part 1 of the PHEP Work Plan for BP3 has been published. The work plan along with updated contact information forms for LHDs and tribes can also be found on the MIHAN in the Document Library at Documents > Local Health > EPC General > PHEP FY 2021-2022 BP3.


Pages 1 – 6 of the work plan contain the usual boilerplate language that encompasses important expectations, federal benchmarks, etc. Pages 7 – 9 contain the activities and deliverables common to both LHDs and tribes. Page 9 contains the activity for tribal health partners only. Page 10 contains an activity for LHDs only, and pages 11 – 13 contain the Cities Readiness Initiative (CRI) activities.

 

Due to the ongoing COVID-19 pandemic and the response activities associated with it, Part 1 contains the minimum activities necessary to meet PHEP requirements.


Local Health Department Situation Report (LHD Sitrep) – Updates

The reporting forms/templates for completed COVID vaccination clinics and planned vaccination clinics have changed three times in the past six weeks. The completed vaccination clinics report form has seen the most changes. The CHECC asks LHDs to make sure they are using the correct version of the report form. The current version has three elements that set it apart from previous versions:

 

     • The street number and street name are in separate columns
     • LHDs are asked whether the MI Volunteer Registry was used to deploy

          volunteers
     • The list of targeted priority populations begins with “healthcare staff” and

         ends with “Other frontline workers.”

 

If the completed vaccination clinics report form you are using is missing any of these elements above, you are using the wrong form.

 

For the planned vaccination clinics report form, two changes were made: the street number and street name were separated into two columns, and the targeted priority populations list is the same as the completed vaccination clinics report form above. If these elements are not in the form you are using, you need to get the updated form.

Applications: Notice of Funding Opportunity (NOFO) to Address COVID-19 Health Disparities

CDC’s Center for State, Tribal, Local, and Territorial Support has published a two-year NOFO: CDC-RFA-OT21-2103: National Initiative to Address COVID-19 Health Disparities Among Populations at High-Risk and Underserved, Including Racial and Ethnic Minority Populations and Rural Communities. Applications are due May 3, 2021. This non-research grant is funded through the Coronavirus Response and Relief Supplemental Appropriations Act, 2021, and is expected to award funding to up to 108 state, local, territorial, and freely associated state health departments, or their bona fide agents. This groundbreaking funding represents CDC’s largest investment to date focusing specifically on reducing health disparities related to COVID-19. It will provide much-needed support to directly address these issues in communities that need it most. For local health department to be eligible they must serve a county population of 2,000,000 or more; or serve a city population of 400,000 or more. Read more about the NOFO and how to apply.
