Change in Local Health Department Situation Report (LHD Sitrep) Vaccination Clinic Report Form

MDHHS Leadership has asked the CHECC to standardize the address information for vaccination clinics reported by LHDs in the weekly sitrep so that geocoding and mapping will be more accurate. Accomplishing this objective required one small modification to the report forms for completed vaccination clinics as well as planned vaccination clinics. In the revised report templates, the street address number is a separate column from the street name.

 

It is imperative that the revised completed vaccination clinic report form, and the revised planned vaccination clinic report form be used beginning with the February 4, 2021, submission. Please replace all older versions of these templates you may have on file with the revised reporting forms.

Mass Vaccination - Sharing of Plans, Procedures, etc.

To facilitate the sharing of ideas, plans, procedures, strategies, etc. around the topic of mass vaccinations during the current pandemic the CHECC has created a folder on the Michigan Health Alert Network (MIHAN) where LHDs and tribal health partners can share these documents. PHEP Deliverables Update

The midyear progress report for BP2 has been waived. Q2 reports for COVID Action Plans and whole community inclusion should be submitted with the Q3 reports due on March 31, 2021.


February PHEP Partners Meeting on Teams

The next PHEP Partners monthly meeting will be a Microsoft Teams Meeting  on Thursday, February 11, 2021.
