The weekly COVID-19 Local Health Department Situation Report (LHD Sitrep) is moving to a new and more robust platform. The questions on the survey are unchanged from last week as is the submission deadline of 3:00 pm Thursdays.
There have been no change in the survey questions. Before you begin the survey, delete your Internet browser's history. Here is the new link to the survey: https://dhhshivstd.iad1.qualtrics.com/jfe/form/SV_6MAgMf7zjzBRlWd
If you find you cannot meet the submission deadline in a given week or you have other questions or concerns, contact Jim Koval, the CHECC Operations Section Chief, at mdhhs-checc-ops@michigan.gov.
The year-end progress report for BP1 has been reduced to one requirement: Share one story of an individual or organization that has brought hope to your community during this time of crisis. How you tell the story is up to you. One submission is required from each LHD and tribal health partners. Submissions are due to mdhhs-betp-depr-phep@michigan.gov by September 30, 2020.
