Thank you for registering to attend the PSD Workshop being held Tuesday, November 5, 2013, at  Western Michigan University's Downtown Campus in Grand Rapids, Michigan.  Registration is at 8:00 a.m. with the program beginning promptly at 8:30 a.m.

Please note:  We cannot accept payment on-site.   Therefore, if you have not yet submitted your payment, please pay online at https://www.thepayplace.com/mi/deq/workshops, or mail a check made payable to the "State of Michigan" to:

CANCELLATION POLICY:  If you do not cancel by 5:00 p.m. three business days before the workshop, you are responsible for the registration fee.
