2026–2027 Seasonal Respiratory

Vaccine Availability, Recommendations, and

Ordering Information

The Maine Immunization Program (MIP) is pleased to announce the following schedule for respiratory virus vaccine availability for the 2026-2027 season:

  • Influenza vaccine ordering is now open.
  • Respiratory syncytial virus (RSV) monoclonal antibody ordering is now open to preposition in locations for an October 1 start date.
  • Maternal RSV and “routine” RSV ordering will be available October 1.
  • COVID-19 vaccine is anticipated to be available in October.

Influenza Vaccine

Availability and MIP eligibility:

MIP will supply seasonal influenza vaccine for all children in Maine, ages 6 months through 18 years. Influenza vaccine for adults 19 – 64 will not be available for the 2026 – 2027 season. However, a limited supply is available for adults 19 – 64 years who are uninsured and underinsured only. This does not apply to adults with private insurance that covers influenza vaccinations.

Vaccine Presentations Available:

Recommendations:

  • Everyone 6 months and older should receive a flu vaccine annually.
  • All 2026-2027 seasonal influenza vaccines are trivalent.
  • All 2026-2027 seasonal influenza vaccines available through MIP are thimerosal-free.

Ordering:

A limited supply of Flulaval and Fluzone will be available to order for adults aged 19 – 64 who are uninsured and underinsured only during the 2026-2027 influenza season. Vaccine orders will be capped at 100 doses per order.

RSV Monoclonal Antibodies

Availability and MIP Eligibility:

MIP will supply RSV monoclonal antibodies for eligible infants and young children. The following products are now available to order for the 2026–2027 RSV season, with no anticipated ordering shortages currently. RSV season runs from October 1, 2026, through March 31, 2027.

 Beyfortus (nirsevimab):

  • 50 mg: All infants <8 months, weighing less than 5 kg (11 lbs.), and born during or entering their first RSV season.
  • 100 mg: All infants <8 months, weighing greater than 5 kg (11 lbs.), and born during or entering their first RSV season.
    • Infants born during April – September should receive a single dose in October.
  • 200 mg: Some children aged 8 months – 19 months who are at increased risk for severe RSV entering theirsecond season, administer as two (2) 100 mg injections.

Enflonsia (clesrovimab):

  • 105 mg: All infants <8 months born during or entering their first RSV season only.
  • Infants born during April – September should receive a single dose in October. 
    • Clesrovimab is not indicated for second season.

Co-Administration with Other Vaccines:

RSV monoclonal antibody can be given alongside routine vaccines, including live vaccines like MMR and Varicella.

Ordering:

  • RSV monoclonal antibody orders must be placed separately from routine vaccine orders in ImmPact.  Orders that combine nirsevimab or clesrovimab with regular vaccines will be denied and must be resubmitted as a separate order. 
  • RSV monoclonal antibody doses are provided to MIP on a monthly allocation basis. Do not bulk order. Place small frequent orders to help minimize waste.
  • Use any remaining unexpired 2025-2026 RSV monoclonal antibody inventory first at the start of the 2026–2027 RSV season.

Storage and Handling – Updated Guidance:

  • Refrigerate at 36°F to 46°F (2°C to 8°C).
  • Both products (nirsevimab and clesrovimab) may be kept at room temperature at between 68°F and 77°F (20°C to 25°C) for a maximum of 48 hours.
  • Once removed from refrigeration, use within 48 hours or discard.
  • Keep in the original carton to protect from light. Do not freeze, shake, or expose to heat.

Maternal RSV Vaccine

Availability and MIP-Eligibility:

Pregnant individuals between 32 and 36 weeks of gestation are recommended to receive a single dose of the bivalent RSV PreF vaccine Abrysvofrom September 1 through March 31, instead of through January 31, to provide adequate protection during the time of circulation of RSV.

It is currently not recommended that pregnant individuals who received the maternal RSV vaccine during their last pregnancy receive an additional dose during a subsequent pregnancy.  For patients vaccinated in a previous pregnancy, their infants should receive a monoclonal antibody.

Recommendations:

Administration of a single, one-time dose to pregnant individuals between 32 – 36 weeks gestational age during September 1 – March 31.

Adult RSV Vaccine

Availability and MIP Eligibility:

MIP will supply RSV vaccine to eligible adults aged 50-64. Ordering will begin October 1, 2026.

Recommendations:

  • RSV vaccination is recommended for all adults 75 years or older and for adults 50-74 who are at increased risk of severe RSV disease.
    • Please Note: State-supplied adult RSV vaccine can only be given to adults aged 50-64 who are uninsured, underinsured, privately insured, or enrolled in MaineCare.
  • RSV vaccine can be administered to adults at any time of year, ideally August through October.
  • RSV is not an annual vaccine. Previously vaccinated adults should not receive an additional dose at this time.

COVID-19 Vaccine

Availability and MIP Eligibility:

MIP will supply COVID-19 vaccines for all children in Maine, ages 6 months through 18 years, starting in October.  COVID-19 vaccines for adults 19 – 64 will not be available for the 2026 – 2027 season.

Recommendations:

The American Academy of Pediatrics (AAP) recommendations for COVID-19 vaccination for children ages 6 months – 18 years include: 

  • All infants and children 6 through 23 months of age
  • Children 6 months through 18 years of age who are moderately or severely immunocompromised
  • Children 2 through 18 years of age in certain additional risk groups, including those at high risk of severe COVID-19
  • Residents of long-term care facilities or other congregate settings
  • Children who have never been vaccinated against COVID-19
  • Children whose household contacts are at high risk for severe COVID-19.
