Subject: 317 Adult Vaccine Pre-book

We are in the process of building a more structured 317 adult vaccine program. If you have an adult population who meets the following criteria, you may be eligible to order.
Patients receiving 317 adult vaccines, MUST be:
- 19 years or older and either:
- Uninsured OR
- Underinsured
Any providers interested in accessing this vaccine will need to fill out their vaccine need for their eligible population within ImmPact. If you do not have patients that fit these criteria, you will not be eligible to order 317 vaccines. Please note that ImmPact is currently down and will be back up at 2:00 PM on Thursday, September 29.
We are also asking that interested providers, complete the following pre-book for adult 317 vaccine orders for October 2022- March 2023. Please respond with your vaccine needs by October 7.
Over the course of the year, we have seen a dramatic uptake in adult 317 vaccine orders. Our funding cycle will end on September 30, and we will start a new cycle with additional funds on October 1. In an effort to better prepare for our budget and the vaccine needs of our providers, we ask that all 317 adult providers complete the survey below.

We will send a survey to you every 6 months to get your estimated vaccine need. Based on these estimates, we will send you a monthly snapshot of how much you have ordered and your remaining balance of estimated doses.
This information will be important to us moving forward to justify the need for additional 317 funds from the Federal CDC. This is NOT an order and you are not guaranteed to receive what you document here. We will do our best to fulfill all vaccine need estimates, however, vaccine availability is contingent on federal funding.
