You are receiving this email because you registered on a COVID-19 vaccine interest list with the Carroll County Health Department. If you have already been vaccinated, please disregard this message.

Our clinics will be open to all Carroll County residents 18+ starting Monday, April 12th, as well as essential workers who work in Carroll County. We will not be sending additional emails to specific groups. However, we want to make sure that high-priority residents are able to get vaccinated as soon as possible, so we are sharing information about current vaccination options:

  • This email contains links and instructions below to register for your first doseof the COVID-19 vaccine at Carroll County Health Department clinics. 
  • Please note: If you have received your first vaccination through the Health Department but have not yet scheduled your 2nd dose appointment, please look through your email and spam folder for an email from "no-reply@multistatep4p.com". You cannot register for your 2nd dose appointment at the clinics below.
  • If you are not able to register for one of the clinics below, please visit our website https://cchd.maryland.gov/registration-links/ in the future for links for other clinics, which will be posted on Thursdays.

Health Department COVID-19 Vaccine Clinic Registration (Moderna only)

Please note we cannot vaccinate people age 16-17 at this time. Only the Pfizer vaccine is approved for this age group; the Health Department currently only offers Moderna. To search pharmacies by vaccination type, visit vaccinefinder.org

Online registration is required for these clinics. If you need assistance, please call our call center at 410-876-4848, open Monday-Friday, 8 am - 5 pm. You may leave a message at any time.

To register:

  • Click the link for the clinic you would like.
  • If you receive an error message, try to copy and paste the link instead of clicking, or try again in a few minutes.
  • If a clinic is full, you will not be able to register. Please note that if it appears there are no appointments available in the appointments window, scroll down for additional times.
  • Choose a time and click continue.
  • Please complete only the required fields on each page, marked by a red star.
  • You will receive a confirmation email, but it may take several hours and may go into your spam folder. It will come from this email address: no-reply@multistatep4p.com
  • Make sure you write down your appointment time and location.

The day of your appointment:

  • Please bring your government-issued photo ID.
  • If you do not have an appropriate ID, you will not be vaccinated.    
  • The clinic will take place inside with proper social distancing.
  • You must wear a face mask over your mouth and nose at all times.
  • There will be seats available while you are waiting for your shot.
  • You may bring one person with you to help you if you need assistance.
  • You will have to wait for 15 minutes in a socially-distanced seating area after receiving the vaccine to be monitored for any adverse reactions.

You will receive a follow up email or phone call with a link to schedule your second dose, about a week to 10 days before your next vaccination should be (28 days after the first shot for Moderna). It will come from the email no-reply@multistatep4p.com – be sure to check your spam folder.
