Important information for individuals under 65 with health conditions regarding the COVID-19 Vaccine in Carroll County, MD
You are receiving this email because you completed the under 65 with health conditions form for COVID-19 vaccination with the Carroll County Health Department.
If you were already vaccinated at another location (mass site, pharmacy, doctor, etc.), and have not let us know, please complete this short form.
We will soon be able to offer vaccination to people age 16-64 with certain health conditions that raise the risk of serious illness from COVID-19, as determined by the Centers for Disease Control and Prevention (CDC).
To qualify, you must have a letter from your healthcare provider stating that you have one of the following conditions that increase your risk of severe illness from COVID-19:

If you meet these criteria, please complete this quick form and we will contact you soon with registration options. You must have ID and a letter from a healthcare provider stating that you have one or more of the above health conditions at the time of vaccination to be vaccinated at a Health Department clinic. If you do not bring these items to the clinic, you will not be able to be vaccinated, even if you have an appointment.
