COVID-19 Vaccination with the Carroll County Health Department

You are receiving this email because you completed the general public form for COVID-19 vaccination with the Carroll County Health Department.

If you were vaccinated at another location (mass site, pharmacy, doctor, etc.), and have not already let us know, please complete this short form.

We will soon be able to offer vaccination to people age 60-64. If you are in this age group, live in Carroll County, and still need to be vaccinated, please complete this short form. You will begin to receive emails with registration information soon. If you do not have email, someone will call you soon with registration information.

If you are not yet age 60-64, you are not yet qualified for vaccination at Health Department clinics. Please continue to follow state and local websites for updates on vaccine eligibility.
