Kentucky Mathematics Improvement Plan Form Available in Infinite Campus
For those districts interested in using Infinite Campus (IC) to track student improvement plans, a new fillable form based on the Mathematics Improvement Plan Template, titled “Kentucky Mathematics Improvement Plan” is available now in IC for use in the 2026-2027 school year with a start date of July 1, 2026. This form mirrors the adaptations to the template found on the Kentucky Reading Improvement Plan Form within IC. Using the form in IC is not required; it is an option that is being provided to districts based on district request.
Adding a Mathematics Improvement Plan to a student’s record in IC allows multiple users to access the same form, allows schools to share the form with parents/guardians through the Parent Portal, and allows the form to travel with a student should they transfer to another school – whether it’s within the district or to another district. Having a record of a student’s Mathematics Improvement Plan in IC allows schools to share their historical intervention record and helps to ensure a more seamless transition for transient students.
Note: Users must be granted access to the PLP module by local KSIS administrators before they can add a Mathematics Improvement Plan form to a student record in IC.

Mathematics Improvement Plan Report Available in Infinite Campus
Similar to the Reading Improvement Plan Report in IC, users will be able to generate the Mathematics Improvement Plan Report in IC for the 2026-2027 school year. The purpose of this report is to allow state, district, and school level leaders to monitor students with Mathematics Improvement Plans. The report will provide a spreadsheet with detailed student level Mathematics Improvement Plan information. This spreadsheet can be filtered on the Form End Date to remove inactive forms.
Note: Users must be granted access by local KSIS administrators before they can see this report and data will not populate on the report until Mathematics Improvement Plan Forms are added to student records during the 2026-2027 school year.
For those not using Infinite Campus to record Mathematics Improvement Plan forms, no action is needed.
