It has been brought to KDE’s attention that, in some district level binders for AKSA Window 1, there was a printing error in the Grade 4 Reading Field Test Form 2 (the wrong grade was included in its place). The field test tasks are found after the operational assessment tasks in each content area and are provided in numerical order (e.g., Form 1, Form 2, Form 3). This error impacts only the district level binder. School level binders are accurate. 

At this time, please ensure that no copies of the Reading Grade 4 Field Test Form 2 are made from the district level binder. DACs will receive a corrected Reading Grade 4 Field Test Form 2 task to replace the incorrect task.  

 Upon receipt of the corrected task, please securely destroy the task currently provided in the place of Reading Grade 4 Field Test Form 2, as the incorrect grade level form is still considered secure test materials. Once the materials have been received and replaced, districts may make copies as needed.
