New Home Delivered Meals Option in Ky.

The Department for Medicaid Services (DMS) is pleased to announce a new option for the Home Delivered Meals service offered through the Home and Community Based (HCB) waiver.

Mom’s Meals is now approved to provide the Home Delivered Meals service in Kentucky. Mom’s Meals delivers nutritious, ready-to-heat meals to an individual’s doorstep. The meals can be refrigerated for up to 14 days. The addition of Mom’s Meals as a provider will expand access to the Home Delivered Meals service for individuals in underserved areas of the state as well as for those with specific nutritional needs. Mom's Meals

During normal waiver operations, the Home Delivered Meals service is only offered through HCB. Due to the COVID-19 pandemic, the Home Delivered Meals service is also temporarily available through the following waivers:

  • Acquired Brain Injury
  • Acquired Brain Injury Long Term Care
  • Michelle P. Waiver
  • Supports for Community Living

For Waiver Participants: If you are in need of the Home Delivered Meals service, talk to your case manager, support broker, or service advisor about adding it to your person-centered plan of care.

For Case Managers/Support Brokers/Service Advisors: To onboard a participant, contact Mom's Meals at (866) 716-3257, option 1, or intake@momsmeals.comOnce Mom’s Meals accepts the participant as a client, the case manager/support broker/service advisor should add Mom’s Meals to the person-centered plan of care as the Home Delivered Meals provider. The case manager/support broker/service advisor should have Mom’s Meals send an email confirming they accept the participant for the Home Delivered Meals service in lieu of a signature on the person-centered plan of care.
