Don't miss your chance to save money on your 2021 health premium!

Health Coaching

Call 855-202-4219 to get started on four easy-to-complete health coaching sessions. You’ll learn how to set and reach important health goals from a certified professional and you’ll be qualified to earn a discount on your healthcare costs next year.

Each call only lasts about 30 minutes! Get started by July 1 to make sure you (and your spouse, if they’re also covered by your health plan) have time to complete the coaching sessions by the Sept. 30 deadline.

Call ActiveHealth at 855-202-4219 to get started, or log in to your ActiveHealth Portal at myactivehealth.com/stateofindiana.

Online Education

Want to earn your discount online? Head to myactivehealth.com\stateofindiana to learn how completing educational activities can help you earn your premium discount in 2021.
