Hi Clerks and staff,

Following up on the theme of Records Management 101, for the rest of January, I will be sending you an email series on the use and application of the most often utilized forms for County and Local Records Management: State Forms 44905 or Notice of Destruction and 30505 or PR-1. These forms were recently updated in 2020 for your convenience. 

When to use the forms:

• SF 44905: Notice of Destruction of County/Local Government Records in Accordance With an Approved Retention Schedule, this form is to be used only for documenting the destruction of scheduled records.

• SF 30505: Request for Exception to County/Local Retention Schedule or Permission to Dispose of Non-Scheduled County/Local Public Records (PR-1), this form is to be used to destroy unscheduled records or transfer the ONLY copy of scheduled public records to the Indiana State Archives or a local historical entity. 

The next two emails will be about the process for both 44905 & 30505 including the approvals needed.
