 Dear Emergency and Immediate/Acute Care Healthcare Providers,
As valued partners in healthcare, we want to reintroduce you to the programs offered by the Lake County Health Department, specifically our Communicable Disease program.
Reporting Responsibilities
As mandated reporters, we rely on your prompt reporting of suspected and confirmed cases of reportable diseases. Our commitment to public health aligns with the Control of Communicable Disease Administrative Code, Title 77. Public Health, Part 690.200.
Timely Reporting Guidelines
Reportable diseases come with specific reporting time frames:
- "Immediate"– within 3 hours (class IA)
- 24 hours (class IB)
- 7 days (class II)
Your vigilance in ensuring timely reports, even while awaiting laboratory results for confirmation, is crucial.
Reporting a Suspected Case
- Call (847) 377-8130, selecting option #1 for the Lake County Health Department's Communicable Disease Program. Our staff is available 24/7 for immediate reportable diseases or to assist with recommendations. For STIs and HIV, call (847) 377-8450 and leave a voicemail.
- Include in the report:
- Disease or condition
- Patient's name, parents' name (if a minor), address, and telephone number
- Date of birth and age
- Gender
- Physician's name, address, and telephone number
- Always adhere to proper infection prevention measures.
Resources for You
Keep a copy of the list of diseases and reporting time frames in your department.
For Sexually Transmitted Infections (chlamydia, gonorrhea, syphilis, Mpox, HIV, and hepatitis C), call the STI Program at (847) 377-8450 and leave a message on the confidential voicemail.
For all other reportable diseases, urgent issues, or assistance with recommendations, utilize our 24/7 reporting hotline at (847) 377-8130, selecting option #1.
Seek our assistance for infection control guidance, testing information, follow-up, rabies post-exposure prophylaxis recommendations, partner services, and treatment assistance (e.g., rabies, Neisseria meningitidis, measles, pertussis, varicella [chickenpox], Ebola, HIV, etc.).

To verify an animal's rabies vaccination status and assess the risk to the client to determine if post-exposure prophylaxis is warranted, call Animal Care and Control at (847) 377-4700. Also, complete our electronic bite report form.

To report an environmental health concern or hazard, call Environmental Health at (847) 377-8020.

To report suspected or confirmed cases of active tuberculosis (TB), call the Tuberculosis Clinic at (847) 377-8700 and leave a voicemail. The clinic also offers TB screening, testing services, and follow-up with a TB provider, if appropriate.

To schedule immunization appointments for patients of the Lake County Health Department, call the Patient Access Center at (847) 377-8800.
