WAUKEGAN, Illinois – Lake County Clerk Robin M. O’Connor issued a reminder today that any Lake County resident hospitalized at election time will still be able to vote. Registered voters who are admitted to a hospital, nursing home, or rehabilitation center within 14 days before the April 2, 2019 Consolidated Election may qualify for personal delivery of a ballot by a family member or friend, subject to statutory conditions.
The voter must complete a request form titled Application for Ballot for Qualified Voter Admitted to a Hospital, Nursing Home or Rehabilitation Center. The voter's physician is required to complete the accompanying Affidavit of Attending Physician.
All required forms must be completed and returned to the Lake County Clerk's office prior to the voter’s Certificate of Application and ballot being issued.
As an alternative, all registered voters may request a ballot by mail until noon on Thursday, March 28. Your ballot can be mailed to any address, and your voted ballot must be returned to the Lake County Clerk’s office with a postmark no later than Election Day.
