Registered Voters Admitted to a Hospital 14 Days Before Election May Vote

WAUKEGAN, Illinois – Lake County Clerk Carla N. Wyckoff issued a reminder today that, even if Waukegan Township and City of Waukegan residents are hospitalized at election time, they will still be able to vote. Registered voters who are admitted to a hospital, nursing home, or rehabilitation center within 14 days before the February 28th Primary Election may qualify for personal delivery of a ballot, subject to statutory conditions.

The voter must complete a request form titled Application for Ballot for Qualified Voter Admitted to a Hospital, Nursing Home or Rehabilitation Center. The voter's physician is required to complete the accompanying Affidavit of Attending Physician. Forms are available by contacting the County Clerk’s Election Department at VotingByMail@lakecountyil.gov or 847.377.2314. Web visitors may also download the Hospitalized Voter Form by visiting LakeCountyClerk.info and clicking on the Forms tab under Resources.

All required forms must be completed and returned to the Lake County Clerk's office prior to the voter’s Certificate of Application and ballot being issued.

As an alternative, all registered voters may submit a request for a ballot by mail until noon on Thursday, February 23. The ballot can be mailed to any address and can be requested by contacting the County Clerk's office at 847.377.2406, votingbymail@lakecountyill.gov, or LakeCountyClerk.info. The hospitalized voter may authorize a spouse or friend to hand-deliver the voted ballot back to the Clerk's office. If the voted ballot is returned by mail, it must have a postmark no later than midnight on Election Day.

The February 28 Consolidated Primary is only for Waukegan Township and the City of Waukegan residents to nominate township highway commissioner, city mayor, and city clerk candidates. No other offices, contested Republican candidates, or nonpartisan public questions were certified for the primary election.
