HIRTA Health Connector Pilot
Danny Schnathorst shared about the new Health Connector pilot HIRTA is developing as part of a USDOT project. The pilot will help riders schedule medical appointments and transportation to those appointments more easily. NaviLens technology further helps guide patients through the hospital.

Health Navigation FY24 Annual Report
Abigail Chihak presented the findings from the Health Navigation program over the last year. She highlighted access to care, food security, and housing cost needs and how navigators were able to assist in these areas. The FY24 Annual Report is now available for download.

Upcoming Workgroup Meetings
Suicide Prevention Coalition September 19 | 3pm | zoom | add to calendar
The Suicide Prevention Coalition will continue the action planning process. Coalition leadership will share their experiences at the Iowa Suicide Prevention Coalition Summit. The group will also discuss potential branding, fundraising, and outreach activities for the upcoming year.
Dallas County Has Heart Workgroup September 24 | 3pm | zoom | add to calendar
The Dallas County Has Heart Workgroup will meet to discuss communications and outreach efforts centering around heart health for the next quarter. The themes for October, November & December are 'Sudden Cardiac Arrest Awareness Month', 'Healthcare Team and Caregivers', and 'Learn Your Health History' respectively.
Attendee Announcements
Suzanne Hegarty, Dallas County Health Department- The emergency preparedness program has assist schools in placing Stop the Bleed kits in every building. Brief online training is available. Several outdoor AEDs will be placed at Triumph park to assist with cardiac events on the grounds.
Jocelyn Monzon, Dallas County Hospital- The Dallas County Hospital hosts the mobile food pantry in Perry every other month. Due to increasing need in the community, the hospital has requested additional food. Because more families are needing food, there is also a need for more volunteers to distribute the food. If you or your organization would like to volunteer, please contact Jocelyn. The next Mobile Food Pantry day at the hospital will be October 23rd from 2:30-3:45.
Natalie Peters, Dallas County Health Department- A new Walk with Ease program will start this October to provide education, exercise, and social connection. The program will be held at the McCreary Center in Perry in collaboration with the Iowa Community HUB. Walk with Ease is geared towards older adults with arthritis. The program will be capped at 15 people. Details on how to register will be coming soon.
Danielle Kenworthy, Short Years Partnership- The Short Years Partnership offers family support and parenting education services and programs in Warren, Dallas, Madison, Jasper, and Marion counties. Programming is available to families in the services areas with children aged birth through three. There is currently a waitlist for Arabic speaking clients. But, thanks to a new full time Parent Educator, there are openings for Spanish speaking clients.
Abigail Chihak, Dallas County Health Department- The Health Department continues to offer free walk in STI testing on the third Wednesday of the month from 3:30-5:00pm. The county is also switching website providers so you will begin receiving meeting notices from a different address. Please check your junk mail if you haven't heard from us in a while. All your current subscriptions will be transitioned for you-no action is needed.

The December Community Health Coalition Meeting will be held at a different location to accommodate our partner's training space needs on the Human Services Campus. We will next meet in person December 12th 9:00-10:30AM at the DALLAS COUNTY LAW ENFORCEMENT CENTER, 28985 Thin Blue Line Lane, Adel. The conference room will be on the right after you enter the main doors.
