Dallas County Releases 2023-2025 Community Health Improvement Plan
Adel, IA - The Dallas County Health Department is excited to announce the Community Health Improvement Plan (CHIP) for 2023-2025. The CHIP identifies key areas of health improvement, outlining actions that individuals, organizations, communities, and policy makers can take to create positive change in our community. Local Boards of Health and nonprofit hospitals in Central Iowa collaborated to complete the Community Health Assessment that provided the Department data and areas of opportunity. Community members provided valuable input to select the top five priorities in Dallas County and to develop the CHIP’s goals, strategies, and target metrics.
For 2023-2025, the Dallas County Health Department identified five priority areas of focus. These include access to care, mental health, nutrition and physical activity, heart disease and stroke, and substance use.
The CHIP provides direction for the Department and community partners to improve the health and well-being of Dallas County residents. It also highlights potential action items for individuals, organizations, communities, and policy makers interested in making health improvements within their own neighborhoods.
