(Adel) - The Dallas County Health Department and Dallas County Hospital and Family Medicine Clinics have identified the health improvement priorities that will be the focus of the 2023-2025 Dallas County Health Improvement Plan.
The top five health improvement priorities are:
- Access to Care
- Mental Health
- Nutrition, Physical Activity, & Weight
- Heart Disease & Stroke
- Substance Abuse
In 2021, Polk, Warren, and Dallas County health leaders worked with PRC, an Omaha based research group, to conduct a regional community health assessment. The regional assessment identified eleven areas of opportunity through community surveys, stakeholder interviews, and public health data analysis. The Dallas County Health Department used these findings to conduct a virtual prioritization workshop, including a presentation of the results and prioritization survey to community partners and residents. The goal of prioritization is to further understand which areas are most important and relevant to those that live or work in Dallas County.
Dallas County Health Department and Dallas County Hospital and Family Medicine Clinics will use these findings to create the 2023-2025 Dallas County Health Improvement Plan, which will contain action steps that individuals, community groups, and agencies can take to improve the health of Dallas County residents. This plan also heavily guides community health programming within the Dallas County Health Department.
Community Health Assessment and Community Health Improvement Planning are completed every three years to identify the most important factors affecting health in the community and to plan strategies to build on strengths and work on gaps.
